Psychological Evaluation Insurance Billing Policies

We are currently accepting all forms of insurance.  We can bill out-of-network if we are not covered by your plan, and you have out-of-network benefits available.

Please provide full insurance information upon your initial phone call to our office to determine your benefits eligibility. We also need to obtain authorization from your insurance provider when necessary prior to your appointment.

If you have a change in insurance, please let us know as soon as possible, so we can ensure payment. We do not schedule appointments until insurance is verified unless you are a private pay client.

Due to cost-sharing with your insurance company, we will do our best to estimate your portion at the time of your appointment. Despite our best efforts, it is possible that once we get the claim back, your cost-share may be higher than originally anticipated. This usually takes 3-6 weeks after it is submitted. We will notify you about any balance due with a monthly statement.

Our billing specialist monitors outstanding credits. In the event that you have overpaid for our services, we will issue you a refund check for your overage.

With new insurance plans taking effect at the start of the each new calendar year, we will be collecting your full visit fee. That fee will be applied to your new deductible at the time of service.

FEES FOR SERVICES

  • Medical clearance evaluations for weight loss surgery or spinal cord stimulator implant surgery typically involve 4.0 clinical hours of the psychologist’s time in administration, scoring, and interpretation of tests. It also requires the psychologist to prepare a written report for your insurance company and doctor.
  • If we will be billing your insurance, the “patient portion” (copay, coinsurance or an applicable deductible) will be due at the start of your appointment.
  • If you are a private pay client, the fee is $450.

ADULT PSYCHOLOGICAL EVALUATIONS

  • The cost for a Comprehensive adult evaluation is determined by the total number of hours required by the psychologist(s) to complete the comprehensive adult evaluation.
  • The total cost will vary between $900 and $1800 depending on the number and selection of tests utilized. For example, a typical neuropsychological evaluation testing battery requires many more hours than other types of adult assessments.
  • If we will be billing your insurance, the “patient portion” (copay, coinsurance or an applicable deductible) will be due at the start of your appointment.

CHILD PSYCHOLOGICAL ASSESSMENT

  • The cost for a comprehensive Child Assessment is determined by the total number of hours required by the psychologist(s) to complete the full evaluation process from testing time with your child to results review session with you.
  • Fees typically vary between $900 and $1800. For example, a typical neuropsychological evaluation testing battery requires many more hours than other types of child assessments.
  • If we will be billing your insurance, the “patient portion” (copay, coinsurance or an applicable deductible) will be due at the start of your appointment.

If paying out-of-network or privately, a partial payment of $500 is required on the day of testing. The balance will be due at your results review appointment.  If your plan does not cover the costs of the type of assessment needed, you will pay for the full cost of the assessment. Then submit the required information to your insurance company for reimbursement to you.

Please note that Legacy does not accept insurance for forensic and child custody evaluations[1]

Cancellation Policy (Private Pay Evaluations)

A credit card is required to be on file in order to schedule psychological testing services. It will only be charged in the case of missed appointments, late cancellation or difficulty obtaining payment after services are rendered.

In order to provide you with optimal care, your appointment time is reserved specifically for you.  Our doctors do not double book.  Your submission of your credit card information allows Legacy to reserve a day of testing for you or for your child.

In return, we ask that you provide us with a minimum of 48 hours advance notice if you are unable to make it to your appointment.  This gives us an opportunity to fill those appointment times with other clients in need of care.  If you provide us with an advanced notice (48 hours) of cancellation, your credit card WILL NOT be charged.  If you are unable to provide us with this notice, you will incur a missed appointment/ late cancellation fee as follows:

In the event that a full day of testing is reserved (6 to 8 hours of testing) and you miss your appointment without notifying our office, your credit card will be charged $500.  This charge is irrespective of the reason for the no-show/missed appointment. Insurance does NOT cover this fee.  However, if you miss your appointment and reschedule the missed appointment within five business days, the psychologist may, at her sole discretion, refund a portion of your cancellation fee.
